At NKC Health, we’re deeply committed to ensuring transparency and easy access to your health information, in alignment with the 21st Century Cures Act. If you ever feel that we’re not meeting the goals of this important act, please don’t hesitate to reach out to our Web Access team — your feedback matters.

If you’d like a copy of your NKC Health patient record, we’re here to help. Simply stop by our Medical Records office on the 1st Floor of 2790 to sign a release form. To make things a little easier, you can also print and fill out the form ahead of time and bring with you.

There is no charge for copies of your medical records when requested for your personal use. We believe your health information should always be within your reach.
If you’re unable to visit the Medical Records office in person, don’t worry. Give us a call at the number below, and we’ll be glad to help make other arrangements that work for you.
